Incoming Freshmen
  • Pass the Admission Test conducted by the Guidance Office and Testing Center
  • Interview and approval of the College Dean and University Registrar
  • Original Form 138 (High School Report Card)
  • Certificate of Good Moral Character
  • Two (2) copies of recent 2x2 colored photographs
  • Medical Certificate issued by an accredited physician
  • Photocopy of PSA/NSO Birth Certificate
  • One (1) Long Brown Envelope
Transferees
  • Certificate of Transfer Credential (Honorable Dismissal)
  • Official Transcript of Records
  • Certificate of Good Moral Character
  • PSA/NSO Birth Certificate
  • Two (2) copies of 2x2 colored photographs
Cross Enrollees
  • Cross-Enrollment Permit signed by the Dean
  • Letter addressed to the University Registrar
  • Certification of subjects to be cross-enrolled
  • One (1) Long Brown Envelope

Graduate School
  • Original Transcript of Records with valid S.O. Number
  • Completed Graduate School Application Form
  • Two (2) copies of recent 2x2 colored photographs
  • PSA/NSO Birth Certificate or Marriage Certificate
  • Certificate of Employment (if applicable)
  • One (1) Long Brown Envelope
Doctoral Program
  • Original Transcript of Records showing completion of Master's Degree
  • Valid Special Order (S.O.) Number
  • Two (2) copies of recent 2x2 colored photographs
  • PSA/NSO Birth Certificate or Marriage Certificate
  • Research Proposal (if required)
  • One (1) Long Brown Envelope

  • Original Transcript of Records showing completion of Bachelor's Degree
  • Valid Special Order (S.O.) Number
  • Certificate of Eligibility to take up Law

Applicants must be at least 4 years old at the start of the school year.

Transfer Pupils

  • PSA Birth Certificate
  • No transfer pupil shall be accepted in Grade 6
