  • Pass the Admission Test conducted by the Guidance Office and Testing Center
  • Interview and approval of the College Dean and University Registrar
  • Form 138 (High School Report Card)
  • Certificate of Good Moral Character
  • Two (2) copies of 2x2 colored picture
  • Medical Certificate from accredited physician
  • NSO Birth Certificate (Photocopy)
  • One (1) Long Brown Envelope
Transferees
  • Certificate of Transfer Credential
  • Original Scholastic Records from previous institution
  • NSO Birth Certificate / Marriage Certificate
  • Two (2) copies of 2x2 Colored Pictures
Cross Enrollees
  • Cross-Enrollment Permit signed by the Dean
